Referral Policy

The myo Clinic accepts patients on referral from a physician licensed to practice medicine in Canada (except Quebec).  After we receive the referral document, you can book your appointment.   Why?

Referrals are valid for six months. A new referral is required if treatment spans more than a six month period.

Availability

We make every effort to book your appointment to minimize a waiting period.  Please check our home page for updates on current wait times.

 

Appointment Frequency

The treating doctor will determine your individualized treatment plan.  The treatment plan defines the number and frequency of your appointments.

 

Changes and Cancellations

If you cannot keep your appointment please advise us by email or phone by 4pm the day before your appointment. Your courtesy in letting us know as soon as possible will allow us to accommodate other patients. Our policy is to charge you for missed appointments.

 

Missed Appointment Policy

We will charge you a missed appointment fee  if you fail to attend the appointment without providing us notice by 4pm the day before the booked appointment.  We will waive this fee if we are able to fill the appointment with another patient on our waiting list.

Patients must pay their missed appointment fee before booking or retaining any further appointments at The myo Clinic.

Missed Appointment Fees are:

• Initial consultation and treatment  $150
• Follow-up appointment   $75
